Multiple Doctors Displayed in Prescription After Reassigning Doctor

Currently, when a doctor is assigned to a patient and later reassigned to another doctor, the prescription section displays multiple doctors instead of showing only the currently assigned doctor.

This causes confusion because prescriptions should be associated with only the latest assigned doctor. However, the system appears to retain the previously assigned doctor along with the new one.

Steps to Reproduce

Assign a doctor to a patient.

Generate or view the prescription.

Reassign the patient to another doctor.

Open the prescription section.

Current Behavior

The prescription shows multiple doctors (previous and newly assigned).

Expected Behavior

The prescription should display only the currently assigned doctor when change consultation is done.

Previous doctor assignments should not appear in the prescription unless explicitly required in history.

Impact

Causes confusion in medical records.

Incorrect doctor association in prescriptions.
