fix(Verify Medicine, Counseling): Display Doctor Name in Consultation Lists

Summary

The doctor's name is not displayed in the consultation selection lists on both the VerifyMedicinesPage and CounselingPage. This creates a poor user experience as volunteers cannot see which doctor conducted the consultation when verifying medicines or providing counseling.
