Fix: add all doctor schema fields to the Add Doctor form

  • Add all doctor schema fields to the Add Doctor form.
  • Required: Name, Age, Sex, Specialization, Email, Phone, License Number.
  • Optional: Availability (default true).
  • Order: Name, Age, Sex, Specialization, Email, Phone, License Number, Availability.
  • Disable “Add to Directory” until required fields are filled.
