Draft: Pre-Health Screening – dashboard entry & patient fetch

Initial frontend groundwork has been laid for the Pre-Health Screening feature. A new Pre-Health Screening entry is available in the Volunteer Actions dashboard, along with a dedicated page that allows patient lookup using the Book Number. Patient details are successfully fetched and displayed using existing APIs. The remaining work involves completing the screening form and submission flow, handling camp-specific screening data, displaying a read-only screening summary during doctor consultation, and refining the UI and edge-case handling.
