add emergency fields and validation to patient registration

This merge request introduces the initial UI foundation for the Emergency Patient Priority feature in the eHRS registration workflow.

Changes Implemented Added an “Is Emergency” checkbox to the patient registration form. Added an “Emergency Type” dropdown with the following categories: Pregnant Elderly Disabled Paralyzed

Remaining Work The following emergency workflow components are not included in this merge and will be implemented separately: Sending emergency data in the patient creation API payload Applying queue priority for emergency patients Performing immediate doctor assignment Updating queue UI to highlight emergency patients
