Fix/change assigning doctor ui

Swarna rathna madhuri Kanda requested to merge fix/change-assigning-doctor-ui into develop

in this i changed the ui of assigned doctor page to change the position of consultation doctor below of the the doctor box and after assign the doctor i add patient name book number doctor name in the confirm assignment
